Perform in-home patient assessments and OASIS documentation Administer medications, treatments, and wound care Educate patients/families on home care and safety Coordinate care with interdisciplinary team Prioritize independent clinical decision-making in patient homes Utilize Home Base Home Care software for documentation and care coordination

Job Qualifications:
