• As an LPN at Epiphany Healthcare, you will play a vital role in delivering exceptional care to our patients.

You will work under the supervision of a Registered Nurse (RN) and collaborate with other healthcare professionals to develop and implement individualized care plans.

Your responsibilities will include:

• Key Responsibilities:
• Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
• Educate patients and their families about health management and disease prevention.
• Maintain accurate and up-to-date patient records and documentation.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in care plan development and implementation.
• Ensure compliance with all healthcare regulations and company policies.
• Qualifications:
• Current LPN license in the state of Georgia.
• Previous experience in home healthcare or a similar setting preferred.
• Strong clinical skills and knowledge of nursing practices.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
