Job Description

JOB SUMMARY
Responsible for providing professional nursing care to clinic patients in the role of triage phone nurse.
ESSENTIAL FUNCTIONS
Triage patient phone calls and walk in patients to assess patient situation/need and respond appropriately and in a timely manner. Inform patient of physician recommendation/intervention; schedule appointment, provide and call in medication to pharmacy is prescribed and/or do referral if needed. Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc. in EMR. Promote wellness by providing patient education materials, communicating physician advice/instructions adheres to HIPPA guidelines and confidentiality policies. Check RX line- contact patient if call from pharmacy and verify RX is needed. Obtain refill approval from provider & e-scribe medicine to pharmacy. Check PDMP website before filling controlled medication Complete prior authorizations as needed.
ADDITIONAL RESPONSIBILITIES
Assist in other areas of the clinic as deemed necessary by clinical supervisor.
QUALIFICATIONS AND SKILLS
State LPN license.
