Sevita is seeking a compassionate Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you'll provide person-centered nursing care, administer medications, monitor health conditions, and support individuals with diverse needs in home and community-based settings. You'll collaborate with an interdisciplinary team to implement care plans, educate individuals and families, and document care accurately. Responsibilities Provide direct nursing care including medication administration, treatments, and monitoring of vital signs Follow individualized care plans and clinical protocols under the direction of RNs and physicians Assist individuals with activities of daily living while promoting dignity and independence Observe, document, and report changes in health status and behavior promptly Support individuals with behavioral health, brain injury, and other specialized needs in home and community settings Educate individuals and families on health conditions, medications, and self-care strategies Accurately document care, interventions, and outcomes in electronic health records (EHR) Maintain infection control, safety, and regulatory compliance at all times Collaborate with an interdisciplinary team to coordinate and optimize care Participate in ongoing training and professional development offered by Sevita Required Skills Medication administration Wound care and basic nursing procedures Vital signs monitoring and basic assessment Electronic health records (EHR) documentation Infection control practices Patient and family health education Time management and organization Interdisciplinary care coordination Basic behavioral health support Home and community-based care practices
