Responsibilities:

Provide direct patient and resident care in accordance with nursing standards and facility protocols Administer medications and treatments as prescribed Monitor resident conditions and report changes to nursing leadership and physicians Assist with assessments and care planning Maintain accurate electronic and written medical records Collaborate with Registered Nurses, CNAs, physicians, and interdisciplinary healthcare teams Promote resident safety, comfort, dignity, and quality of life

LPN Requirements:

Active Iowa Licensed Practical Nurse (LPN) License Minimum of one (1) year of recent Long Term Care (LTC), Skilled Nursing, Nursing Home, or Rehabilitation experience Current American Heart Association (AHA) BLS Certification Strong clinical assessment and communication skills Ability to work independently and collaboratively within a healthcare team Successful completion of background screening and credentialing requirements
