Job Description
Keystone Housecall, Idaho's leading housecall provider since 2013, is currently seeking a Licensed Practical Nurse for our Nurse Case Manager position. The successful candidate possesses a current LPN license and exhibits an outgoing, professional, and compassionate presence. Our nurses contact patients both in person and via phone to quality of care, as well as establishing and maintaining relationships with hospitals, assisted living facilities, and skilled nursing facilities. This role works closely with providers and non-clinical team members to coordinate and deliver the level of care expected of our team. Keystone is changing geriatric care in Idaho with unsurpassable clinical care provided by our dedicated and specialized team. Requirements:
- Current Idaho LPN license
- Current driver's license
- Clean background check and driving record
- Reliable Transportation with appropriate insurance
- One year of med/surg or geriatric experience preferred, however we will consider new graduates.
