RESPONSIBILITIES
As the Licensed Practical Nurse (LPN), you will play a vital role in coordinating patient care and supporting our team of healthcare professionals who provide medical services in patients' homes and community settings. Working from our office, you will serve as a primary point of communication for patients, families, caregivers, community partners, and healthcare providers, ensuring questions and concerns are addressed promptly and appropriately. The LPN works closely with our Nurse Practitioners and other clinical team members to coordinate patient visits, prioritize care needs, process referrals, and facilitate communication between all members of the care team. Responsibilities include documenting patient information in the electronic medical record, processing medication requests and refills as directed, coordinating laboratory services, durable medical equipment (DME), and other clinical resources, faxing and tracking provider orders, providing patient and caregiver education, assisting with care coordination, and supporting the timely completion of clinical documentation. The ideal candidate is organized, compassionate, detail-oriented, and committed to delivering exceptional patient-centered care in a collaborative community healthcare environment.
RN REQUIREMENTS
Current LPN license to practice in Idaho Minimum of one (1) year nursing experience is preferred. Experience in hospice is also preferred. Current First Aid and CPR certification Schedule flexibility and local travel required Ability to communicate effectively verbally and in writing Must have valid drivers license, vehicle insurance, and reliable transportation #
