Job Description

MDS Nurse (RN/LPN) Mercer Manor Rehabilitation is seeking an MDS Nurse (RN/LPN) to join our skilled nursing facility. This is a full-time, salaried opportunity offering competitive compensation, performance incentives, comprehensive benefits, and a supportive leadership team. Previous MDS experience is preferred, but we are willing to train the right RN or LPN with skilled nursing or long-term care experience.
Compensation RN:
Up to $90,000 per year
LPN:
Up to $80,000 per year Salary growth is tied to facility census. Plus, earn up to $7,500 annually through our Quality Measure Bonus Program! Responsibilities Coordinate, schedule, and complete MDS assessments. Ensure timely and accurate MDS documentation. Collaborate with the interdisciplinary team to support quality resident care. Help maximize appropriate reimbursement through accurate assessments. Maintain compliance with state and federal regulations. Qualifications Active Illinois RN or LPN license required. Previous MDS experience preferred; training is available for the right candidate. Skilled nursing or long-term care experience required. Strong organizational and communication skills.
