In keeping with the Mission, Vision, and Corporate Values of Heritage Behavioral Health Center, the Licensed Practical Nurse I

•Integrated Care provides nursing services within Heritage's Integrated Health Clinic, which delivers primary and psychiatric healthcare services to individuals with serious mental illness and chronic health conditions. This position plays a vital role in ensuring patients receive safe, timely, and compassionate care. The LPN I works under the direction of the RN Program Manager and alongside the LPN II team members, who provide day-to-day workflow coordination, guidance, and mentoring. This position collaborates closely with providers, nurses, medical assistants, pharmacy partners, and behavioral health staff as part of an integrated care team. This position involves regular, direct contact with individuals living with mental illness, substance use disorders, and chronic health conditions. A compassionate, professional, patient-centered approach is essential.

Communicate proactively with the RN Program Manager regarding patient concerns, clinic flow, and scheduling

Remain open to guidance, mentoring, and real-time workflow support from LPN II staff and leadership

Collaborate effectively with providers, team members, and community partners

Follow all Heritage policies, procedures, safety standards, and infection control practices

Core Responsibilities:

Clinical Care & Patient Support Accurately obtain and document vital signs, including blood pressure and other key indicators

Administer prescribed medications (weekly or as ordered) and report any adverse reactions to medical staff

Monitor health status and ensure timely, accurate charting in medical records

Communicate changes in patient condition clearly to providers

Provide compassionate, respectful, and professional support to patients and families

Maintain readiness of clinical supplies, medications, and equipment Care Coordination Coordinate with pharmacy partners to ensure timely, accurate medication refills and adjustments

Facilitate referrals and link patients to appropriate healthcare services

Assist with triage, assessment, and coordination of care for urgent health concerns by collaborating with healthcare professionals

What Success Looks Like:

The Licensed Practical Nurse I will be successful in this role when: Patients receive timely, coordinated, and compassionate nursing care within their scope of practice

Vital signs, medication administration, and health monitoring are documented accurately and on time

Changes in patient condition are communicated to providers promptly and clearly

Medication refills, prior authorizations, and pharmacy coordination are handled accurately and without delay

Clinical supplies, medications, and equipment are consistently ready for each clinic day

Providers and team members view this person as reliable, responsive, and communicative

Knowledge, Skills and Abilities:

Ability to engage individuals and reduce barriers to accessing care

Strong problem-solving, multitasking, and activity-coordination abilities

Excellent interpersonal, verbal, and written communication skills

Ability to work effectively in a team-based environment

Strong customer service skills

Ability to respond effectively in emergency situations

Attentive to detail with strong listening and communication skills

Excellent organizational skills; able to work independently and as part of a multidisciplinary team

Compassionate and empathetic approach centered on patient well-being

Education and Experience:
Required:

Active, unencumbered State of Illinois LPN license

Current CPR certification, or ability to obtain upon hire, with certification maintained throughout employment

Valid driver's license, reliable transportation, and proof of current automobile insurance

Preferred:

Experience in behavioral health, integrated care, primary care, or community health settings

Experience serving individuals with serious mental illness or co-occurring conditions

Note:

Equivalent combination of education and experience will be considered.

Physical Requirements and Working Conditions:

Ability to stand, walk, and move throughout the clinic for extended periods, with frequent patient interaction

Ability to safely assist and support patients, including during urgent medical situations

Potential exposure to bloodborne pathogens and other clinical environments; adherence to universal precautions and proper use of personal protective equipment required

Regular contact with individuals experiencing serious mental illness, substance use challenges, and chronic health conditions

Ability to respond calmly and effectively in crisis, code medical, or emergency situations

Valid driver's license, reliable transportation, and proof of current automobile insurance required

Ability to obtain and maintain CPR certification
