Responsibilities Deliver comprehensive nursing care to pediatric patients in their homes, including assessments, medication administration, and vital signs monitoring. Implement individualized care plans tailored to each child's unique needs, including IV infusions, tube feedings, and ventilator management. Coordinate with families and caregivers to educate them on treatment protocols, medication management, and safety procedures. Document all patient interactions accurately using Electronic Health Record (EHR) systems such as Epic or eClinicalWorks, ensuring compliance with HIPAA and confidentiality policies. Conduct clinical evaluations during visits, including physical examinations and diagnostic evaluations for developmental disabilities or chronic conditions like Alzheimer's or dementia when applicable. Manage acute pain episodes and assist with emergency interventions such as airway management or sterile procedures like suturing or catheterization. Collaborate with interdisciplinary teams including physicians, therapists, and case managers to ensure seamless continuity of care across settings. Requirements Valid LPN or RN license in good standing; additional certifications such as Basic Life Support (BLS) and Pediatric Advanced Life Support (PALS) are preferred. Demonstrated experience working with children in settings such as home care, hospital environments—including ICU, PICU, or emergency rooms—or skilled nursing facilities. Critical care experience or ICU background is highly desirable for managing complex cases involving acute pain management or ventilator support. Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks; proficiency in ICD coding (ICD-9/10) and medical documentation standards. Strong knowledge of physiology, anatomy, medication administration techniques including injections and venipuncture, and infection control practices. Experience working with individuals with developmental disabilities or chronic illnesses such as dementia or Alzheimer's disease is a plus. Excellent caregiver communication skills with the ability to educate families effectively while maintaining clinical confidentiality policies. Ability to work independently in a home environment while adhering to long-term care regulations and safety protocols.
