Job Description

POSITION OVERVIEW
Provide direct, compassionate nursing services to individuals, families, special groups, and schools in clinic, and community settings. This role combines hands-on patient care with referrals, documentation, care coordination, and operational support for the Community Health Center.
KEY RESPONSIBILITIES
Deliver professional nursing care under physician orders and established Department and Community Health Center standards. Perform injections, blood draws, nursing assessments, and other clinical procedures while following biohazard and OSHA Bloodborne Pathogen precautions. Coordinate referrals and complete timely patient-care, billing, grant, and statistical documentation. Serve patients with courtesy, cultural awareness, confidentiality, and a nonjudgmental approach. Support front-desk functions, including scanning and scheduling, and assist with phones, computers, and office equipment. Monitor and reorder clinical supplies and keep examination rooms appropriately stocked; perform other assigned duties.
MINIMUM QUALIFICATIONS
Graduate of an accredited school of nursing with a current Illinois Licensed Practical Nurse license. Knowledge of professional nursing theory and practice and a strong interest in community health. Valid driver's license and ability to drive a vehicle.
SKILLS AND ABILITIES
Skilled, professional performance of clinical nursing procedures and patient assessments. Sound independent judgment; ability to organize, prioritize, and manage multiple daily assignments. Ability to work effectively with minimal supervision, both independently and as part of a team. Strong therapeutic relationships with patients and professional relationships with coworkers and partner agencies.
