Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality, patient-centered nursing care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves delivering essential nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. The ideal candidate will possess a strong foundation in clinical skills, excellent communication abilities, and a commitment to enhancing patient well-being through attentive care and professional expertise. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or memory care requirements. Support acute pain management strategies and participate in emergency response activities like first aid and airway management. Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations. Manage patient documentation meticulously while maintaining strict adherence to HIPAA and clinical confidentiality policies. Conduct patient education on medication administration, health coaching, and post-discharge instructions to promote recovery and wellness. Assist with tube feeding procedures, wound suturing, sterile processing, and other technical nursing tasks. Support triage activities in urgent care or emergency room settings by prioritizing patient needs efficiently. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the respective state or jurisdiction. Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics. Demonstrated proficiency in EMR/EHR systems like Epic, Cerner, or eClinicalWorks; familiarity with
ICD-9/10
coding is preferred. Critical care experience including ICU or emergency medicine is highly desirable. Knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to nursing practice. Experience working with individuals with disabilities or those requiring specialized dementia or memory care services. Ability to perform technical procedures such as phlebotomy, venipuncture, catheter care, ventilator management, and sterile techniques. Strong communication skills for effective caregiver communication and client interaction across diverse populations. Certification in Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR). Additional experience in home & community care, pediatric or neonatal ventilator management is advantageous.
