This position is supervised by the Human Resources Department and the Residential Unit Coordinator when appropriate and is responsible for providing comprehensive health care, intervention, and outreach services according to standards set forth by Illinois DMHDD, CARF, and the Executive Director. This responsibility includes self-medication training plans, psychiatric/medical consultation when suggested, direct-service in support of other agency programs, medication review, medical outreach, and participation in the development/implementation of safety procedures/practices. The Agency Nurse participates in individual habilitation planning as a member of the Interdisciplinary Team. Two years direct service (volunteer or paid employment) in rehabilitation, residential services, counseling, medical services or related area and/or supervisory experience in this or related field.

Ability and knowledge to relate to disabled and geriatric individuals in a positive and effective way. Sincere commitment to confidentiality, client services and professional representation to the Center. Must be able to relate to individuals, agencies, etc., in a positive, tactful, and effective manner. Be friendly, courteous, cooperative and service oriented. Must be able to work cooperatively and professionally with all agency staff on a consistent basis. Must be able to provide own transportation to and from work. Must provide proof of adequate personal vehicle insurance. Ability and willingness to address unscheduled emergency job responsibilities during non-work/personal time in a positive manner. Willingness and ability to maintain accurate, timely, and confidential records and/or reports. Must be able to read, write, and speak the English language in a correct, consistent, and understandable manner. Must have demonstrated strength in communication skills and organizational abilities. Must be able to follow oral and/or written instructions.
Pay:
$28.00 per hour Expected hours: 30.0 - 45.0 per week
