Wellness Coordinator (part-time) - Community Living New Horizons is looking for a Wellness Coordinator employee (Nurse) to assist adults with intellectual and developmental disabilities to reach their goals, grow, develop skills and live as independently as possible in the community. Additionally, a Wellness Coordinator is responsible for the development, maintenance and routine monitoring of the individual waiver participant s wellness plan and the medical services required to manage the health care needs of the individual. A successful Wellness Coordinator employee will have excellent communication and interpersonal skills to foster a positive relationship with the individual(s) they support. The Wellness Coordinator will need to utilize critical thinking, time management, and organizational skills while supporting the individual(s) they support. A Wellness Coordinator employee makes a positive difference in the life of the individual(s) they support. Job Qualifications Nursing degree from an accredited school is required Have and maintain a valid nursing license (RN or LPN) Valid Driver s License Complete required training including First Aid/CPR certification Additional Information Drug Test required Background report and driving record required TB test required
