The ideal candidate will provide compassionate, high-quality nursing care to patients in their homes, supporting recovery, managing chronic conditions, and promoting overall well-being. This role requires a thorough understanding of patient assessment, medication administration, and specialized care techniques across various medical settings. Experience with EMR/EHR systems such as Epic, Cerner, and Athenahealth is preferred. The LPN will collaborate with interdisciplinary teams to ensure comprehensive patient care, including long-term care, hospice, and acute care services. A strong knowledge of medical terminology, anatomy, physiology, and infection control standards is essential for success in this role. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history documentation. Administer medications and injections safely following prescribed protocols. Perform IV infusions, catheterization, wound care (including suturing), and sterile procedures adhering to aseptic techniques. Manage airway devices such as ventilators and assist with respiratory support as needed. Provide specialized care for patients with dementia, Alzheimer's disease, memory impairments, or behavioral health needs. Support patients with complex conditions such as acute pain management, infectious diseases, or post-operative recovery. Educate patients and families on health coaching, medication management, nutrition, and discharge planning. Document all patient interactions accurately within EMR/EHR systems like Epic or Cerner while maintaining HIPAA compliance. Collaborate with physicians and other healthcare professionals in outpatient clinics or home & community settings to coordinate ongoing care plans. Assist with diagnostic evaluations including specimen collection/processing and medical imaging procedures when necessary. Ensure infection control protocols are followed diligently across all patient interactions and procedures. Experience Prior experience in diverse healthcare environments such as hospital medicine (ICU, ER, trauma centers), nursing homes, assisted living facilities, or home care is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, eClinicalWorks, or Athenahealth. Experience administering injections (including IV insertion), phlebotomy, tube feeding management, and ventilator support preferred. Knowledge of ICD coding (ICD-9/10), CPT coding for procedures and treatments is advantageous. Background working with special populations such as pediatrics (neonatal ventilator), geriatrics (dementia/cognitive impairments), or individuals with disabilities enhances candidacy. Familiarity with infection control practices and sterile processing techniques required for safe patient care delivery. Certifications in Basic Life Support (BLS) and other relevant nursing credentials are mandatory.
