Job Summary We are seeking a compassionate and highly skilled Licensed Practical Nurse (LPN) to join our dedicated healthcare team. The ideal candidate will possess extensive experience in various clinical settings, demonstrating expertise in patient care, medication administration, and medical documentation. As an LPN, you will provide essential nursing support, ensuring the delivery of safe, effective, and personalized care to diverse populations including seniors, individuals with developmental disabilities, and those requiring specialized medical attention. Responsibilities Perform comprehensive patient assessments by monitoring vital signs and evaluating physical conditions to inform care plans. Administer medications and injections accurately while adhering to prescribed protocols and safety standards. Assist with medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes. Maintain detailed and compliant medical records using electronic health record (EHR) systems like Epic or Cerner. Support patient mobility and comfort through proper use of assistive devices such as Hoyer lifts and ventilators. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to patient needs. Educate patients and their families on health management strategies, including medication adherence and chronic disease management. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals, or home health settings. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation purposes. Knowledge of medical terminology, anatomy, physiology, and infection control practices. Experience with specialized care areas such as Alzheimer's/dementia care, hospice support, acute pain management, or trauma centers is highly desirable. Strong communication skills with the ability to effectively interact with patients of all ages and backgrounds. Certification in Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR). Ability to work independently while maintaining compliance with HIPAA regulations and confidentiality policies.
