The Weekend Option LPN plays an essential role in delivering compassionate, patient-centered nursing care under the direction of the attending physician and Registered Nurse, in accordance with the individualized plan of care. With a focus on comfort, safety, and clinical excellence, this nurse supports patients and their families during critical weekend hours, helping them navigate their hospice journey with dignity and support.
Key Responsibilities:
Contributes to the ongoing assessment and re-evaluation of patient needs and reports changes in condition to the RN and/or attending physician Provides direct, hands-on nursing care to patients during weekend hours Demonstrates competency in essential hospice nursing skills such as pain and symptom management support, wound care, medication administration, infusion therapy (as permitted), and aseptic technique Responds appropriately to patient changes, emotional crises, and end-of-life situations, including making on-site visits as needed Serves as an advocate for patients and their families, helping ensure their needs, choices, and goals of care are respected Builds and maintains positive, supportive relationships with patients, caregivers, and interdisciplinary team members Assists in educating patients and families regarding comfort measures, medications, and Advance Directives Communicates changes in condition or care needs to the RN Case Manager, attending physician, and interdisciplinary team Follows agency policies and complies with state and federal regulations related to hospice care
Requirements:
Reliable transportation and a valid driver's license Current LPN licensure in the State of Indiana Previous hospice, home health, or healthcare experience preferred Strong communication, time management, and critical thinking skills Compassionate and patient-focused mindset Guardian Angel Hospice is a premier, locally owned hospice organization serving 30 counties.
