Job Overview We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess strong clinical skills, a passion for patient-centered care, and experience working in various healthcare settings. As an LPN, you will provide essential nursing services, support patient assessments, and assist in delivering high-quality care tailored to individual needs. Responsibilities Administer medications and injections accurately following physician orders and established protocols. Conduct patient assessments, monitor vital signs, and document clinical observations in accordance with medical documentation standards. Assist with wound care, catheterization, IV infusion, and other clinical procedures utilizing aseptic techniques. Collaborate with multidisciplinary teams to develop and implement personalized care plans for patients with diverse needs. Provide support for patients with complex conditions such as Alzheimer's disease, dementia, or developmental disabilities. Educate patients and their families on health management strategies, including medication administration and lifestyle modifications. Maintain compliance with HIPAA regulations and ensure the confidentiality of all medical records and sensitive information. Requirements Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience in medical-surgical settings, long-term care facilities, or home health environments. Knowledge of EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation. Experience working with geriatric patients and individuals with disabilities or chronic conditions such as dementia or traumatic brain injury. Proficiency in clinical procedures including venipuncture, phlebotomy, tube feeding, and airway management. Strong understanding of infection control practices and long-term care regulations. Excellent communication skills with the ability to provide compassionate patient care while collaborating effectively with team members.
