Role:

As an LPN, you will be responsible for providing direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs). You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each resident. Responsibilities:

Provide direct patient care, including administering medications, performing vital signs, and assisting with ADLs Develop and implement individualized care plans in collaboration with the healthcare team Conduct wound care, administer oxygen therapy, and perform phlebotomy as needed Communicate effectively with residents, families, and healthcare team members to ensure quality care Document resident care accurately and thoroughly in accordance with facility policies Participate in quality improvement initiatives to enhance resident outcomes Collaborate with other healthcare professionals to address resident needs and concerns
