Residential Care Coordinator-Day Shift LPN- Gran Villas- JOB SUMMARY:
Responsible for coordinating patient care specific to meet patient need, and needs for patient outcome, cost, and communication, under the direction and supervision of the Assisted Living Operator.JOB RESPONSIBILITIES
Assumes responsibility for coordinating plans for the total care of each resident which comply with physician's orders, governmental regulations, and facility resident care policies.Confers daily with the facility director regarding current status of nursing services to residents and submits written reports as requested.

MINIMUM QUALIFICATIONS AND RELEVANT EXPERIENCE REQUIRED
Current registration as an LPN for the state in which the facility is locatedPreferred one year experience in the long term care fieldPreferred supervisory or management role in health care field.Possess leadership skillsStrong written and verbal communication skills #IND1
