Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with diagnostic evaluations such as specimen collection, lab processing, and medical imaging procedures. Manage complex care needs including wound care, catheterization, tube feeding, ventilator management, and pain control strategies. Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA and confidentiality policies. Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure seamless patient transitions across care settings. Provide specialized care for patients with conditions such as dementia, Alzheimer's disease, developmental disabilities, infectious diseases, or those requiring hospice or palliative care. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience in diverse clinical environments such as hospitals (including ICU or trauma centers), nursing homes, skilled nursing facilities, or home care settings. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and medication administration protocols. Experience working with geriatric patients and individuals with disabilities or complex health needs such as dementia or memory care. Ability to perform clinical procedures including phlebotomy, suturing, sterile processing techniques, catheterization, and first aid. Excellent communication skills for caregiver interaction and patient education while adhering to HIPAA regulations. Critical thinking skills with experience in triage assessments and acute pain management in fast-paced environments such as emergency rooms or urgent care clinics.
