Overview We are seeking a dedicated Licensed Practical Nurse (LPN)(6p-6a) to join our healthcare team, providing compassionate and comprehensive patient care across various settings. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in electronic health records systems such as Point Click Care. This role offers opportunities to work in long-term care, hospice. The LPN will play a vital role in patient assessment, medication administration, wound care, and supporting interdisciplinary teams to ensure optimal patient outcomes. Candidates with experience in geriatrics, dementia care, and infection control are highly preferred. Duties Perform comprehensive patient assessments including vital signs and physical examinations Administer medications and injections accurately following physician orders Catheter care with aseptic technique Assist with diagnostic evaluations such as blood draws (phlebotomy), specimen collection/processing, and laboratory testing Provide wound care, suturing support, and sterile procedures in accordance with protocols Monitor patients' conditions using physiology knowledge and recognize signs of acute pain or deterioration Document all patient interactions thoroughly within EMR/EHR systems ensuring HIPAA compliance Support discharge planning and educate patients on post-care instructions including medication management and nutrition Participate in infection control practices and maintain compliance with NCQA standards Assist with specialized procedures like spinal taps, gastric lavage, or respiratory support including ventilator management when required Provide compassionate care for vulnerable populations including individuals with developmental disabilities, Alzheimer's/dementia/memory care needs Requirements Valid Licensed Practical Nurse (LPN) license in the state of practice Proficiency with EMR/EHR systems including Point Click Care Knowledge of

ICD-9/ICD-10
