Job Description
Home Health RN/LPN Community Care Connections has an immediate opening for an RN/LPN who is responsible for coordinating and providing patient care. Community Care Connections is a member of the Medicalodges, Inc. family. Medicalodges offers a continuum of health care options which include in-home services, independent living, assisted living, skilled nursing home care, rehabilitation, specialized care, outpatient therapies, adult day care, as well as services and living assistance to those with developmental disabilities.
Coordinating patient care needs.
Assisting in transfers and discharges of patients.
Coordinating with community resources to provide equipment to meet the needs of the patient.
Evaluating support systems for the patient and providing the names of other support systems required/needed. Coordinating with other support systems to meet patient needs.
Physical care of assigned patient.
Obtaining/writing/interpreting doctor's orders and the patient's plan of care.
Coordinating with other disciplines and/or the Case Manager's plan of care to meet patient needs.
Documenting wellness monitoring visit with Case Manager.
Timely submission of all required documentation.
JOB RESPONSIBILITIES
