Full Time/ PRN 12 hour shift

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN/ LVN) to join our healthcare team. As a LPN/LVN you will be responsible for providing high-quality, patient-centered care to residents and ensuring a safe and supportive environment. You will play a vital role in enhancing the quality of life for our patients by delivering excellent clinical care, managing patient interactions, and collaborating with other healthcare professionals. Essential Duties:
Administer medications, treatments, and wound care as prescribed by physicians. Monitor and assess patient conditions and promptly report changes. Assist in patient admissions, discharges, and transfers. Provide compassionate care, ensuring patients' comfort and dignity. Maintain accurate patient records and documentation in accordance with facility policies. Coordinate with doctors, CNAs, and other healthcare team members to deliver comprehensive care. Educate patients and families on health management and discharge instructions.
Work Experience Requirements:
Current LPN/LVN license in [State] Strong clinical and patient management skills Excellent communication and interpersonal skills Ability to work in a fast-paced environment and handle challenging situations with grace Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) preferred
