Responsibilities:

Provide direct nursing care to residents in accordance with care plans Administer medications and treatments as prescribed Monitor residents' health status and report changes to the care team Collaborate with CNAs, RNs, and interdisciplinary team members Ensure a high standard of care focused on dignity, comfort, and well-being Maintain accurate and timely documentation

Qualifications:

Current LPN license in the state of Kentucky Strong clinical skills and attention to detail Excellent communication and interpersonal abilities A genuine passion for caring for others Long-term care experience preferred, but not required What Makes You a
