Job Description

Encompass Health Cardinal Hill Rehabilitation Hospital is seeking a dedicated LPN/LVN to join our Healthcare & Medical Services team. In this role, you will provide high-quality, compassionate care to rehabilitation patients, administer medications, monitor patient progress, and collaborate closely with an interdisciplinary team. Responsibilities Provide direct bedside nursing care to rehabilitation patients within the LPN/LVN scope of practice Administer prescribed medications and treatments, monitoring patient responses and documenting accurately Measure and record vital signs, intake and output, and other key clinical indicators Perform routine clinical procedures such as wound care, catheter care, and dressing changes as directed Assist with patient assessments and promptly report changes in condition to the RN or physician Support safe patient mobility, transfers, and activities of daily living in coordination with therapy goals Educate patients and families on care plans, medications, rehabilitation goals, and discharge instructions Maintain accurate and timely documentation in the electronic medical record (EMR) Adhere to infection prevention, fall prevention, and other patient safety protocols at all times Participate in interdisciplinary team meetings, shift handoffs, and quality improvement initiatives Collaborate effectively with RNs, therapists, physicians, and support staff to optimize patient outcomes Demonstrate compassionate, respectful communication and uphold organizational values of integrity and teamwork Required Skills Medication administration and dosage calculation Vital signs monitoring and basic patient assessment Wound care and dressing changes Catheter care and basic continence management Rehabilitation-focused patient care and mobility assistance Electronic medical record (EMR) documentation Infection prevention and control practices Basic life support and emergency response Patient and family education skills Time management and task prioritization Interdisciplinary team communication and collaboration Understanding of rehab diagnoses and care plans
