Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will help ensure patients receive personalized, efficient, and safe nursing services. Our 10/6 shift works a schedule of 4 on and 2 off Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health records (EHR) We use Point Click Care Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Assist with diagnostic evaluations, specimen collection and processing, and wound suturing as needed. Monitor patients with complex needs such as dementia or Alzheimer's disease. Provide specialized care for patients with acute pain, infectious diseases, or requiring hospice or palliative care services. Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices in geriatrics and pediatrics. Educate patients and their families on health management, medication administration, nutrition, and post-discharge care instructions to promote recovery and well-being. Experience Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state. Proven experience working in diverse healthcare settings such as hospitals (including Level I Trauma Centers), nursing homes, skilled nursing facilities, outpatient clinics, or home health environments. Experience working with individuals with disabilities or developmental delays; knowledge of dementia care and memory support strategies is advantageous. Skills in IV insertion, phlebotomy, medication administration (including injections), tube feeding management, and infection control procedures. Strong understanding of medical terminology, anatomy knowledge, ICD coding (ICD-9/10), CPT coding for procedures, and long-term care regulations. Ability to communicate effectively with patients of all ages—including pediatric and neonatal populations—and collaborate within multidisciplinary teams to deliver exceptional patient-centered care.
