Full Job Description Job Summary Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) Intake Nurse specializing in Home Health services! In this vital role, you will be the first point of contact for new patients, conducting comprehensive assessments, gathering essential medical information, and coordinating initial care plans. Your energetic approach and compassionate communication will ensure a smooth onboarding process for patients requiring home-based nursing care. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working within a supportive and innovative environment. Duties Obtain thorough patient intake referral information from referral sources. Collect and document detailed clinical information using Electronic Health Record (EHR) systems such as Epic and Athenahealth Coordinate with interdisciplinary teams to develop personalized care plans tailored to each patient's needs Communicate effectively with patients, families, and caregivers to explain procedures, answer questions, and provide education on home health services Assist ith tracking all disciplines, schedules, and procedures on all patients Ensure compliance with HIPAA regulations and maintain strict confidentiality of all patient information Support ongoing patient monitoring by triaging symptoms, assessing acute pain or infectious disease concerns, and escalating issues appropriately Telephone professionalism at all times. Attention to detail and must be highly organized due to the fast pace and multiple tasking required for this job. Experience Valid LPN license with current state licensure required Prior experience working in home health care or medical office settings preferred Critical care experience such as ICU or emergency medicine enhances patient assessment skills Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks is highly desirable Experience working with diverse populations including geriatric patients, individuals with developmental disabilities, dementia or Alzheimer's care experience is a plus Knowledge ofICD-9/ICD-10
coding, medication administration, and clinical documentation standards necessary for accurate record keeping. Strong communication skills for caregiver and patient interaction as welll as Physicians and referral sources.
