Job Description
Role Overview The Wellness Nurse (LPN/LVN) provides compassionate, respectful care to senior residents, supporting their health and well-being. Responsibilities include conducting wellness assessments, monitoring health changes, managing medications, and ensuring clinical compliance. The role involves communicating with residents, families, physicians, and care teams, as well as documenting care activities accurately. Additional duties include incident and fall monitoring, supporting infection control, participating in quality initiatives, and training care team members. Qualifications & Skills Active LPN/LVN license At least 1 year of experience in senior living or clinical setting Strong assessment, documentation, and medication administration skills Excellent communication and problem-solving abilities
