Job Description
Job Summary Montgomery Community Health Center is seeking a dedicated and skilled Licensed Practical Nurse (LPN) to provide compassionate, high-quality patient care. The ideal candidate will possess a strong foundation in nursing practices, with experience in community care environments. As an LPN, you will play a vital role in supporting patient health through comprehensive assessments, medication administration, and collaborative care planning. Your expertise will contribute to creating a safe, respectful, and healing environment for all patients. Responsibilities Perform detailed patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following established protocols and physician orders. Monitor patients' responses to treatments and communicate changes promptly to the healthcare team. Document all patient interactions and clinical activities thoroughly within electronic health record (EHR). Maintain compliance with HIPAA regulations and infection control standards to safeguard patient confidentiality and safety. Skills Dependability Proficiency in EMR/EHR systems for accurate documentation. Strong knowledge of medical terminology, anatomy, and physiology. Experience working with diverse populations including pediatrics, geriatrics, and individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease. Skilled in medication administration including injections, sterile processing techniques, collecting samples, and performing lab testing as assigned while demonstrating excellent patient care skills. Excellent communication skills to collaborate effectively. Knowledge of infection control practices and advanced skills in pediatrics are advantageous for this role.
