We are seeking a Licensed Practical Nurse (LPN) to join our Family Medicine clinic. The ideal candidate is compassionate, detail-oriented, and thrives in a collaborative, team-based healthcare environment. Position Summary The LPN supports providers in delivering comprehensive primary care services. This role is essential in ensuring efficient clinic flow, quality patient care, and positive patient experiences. the LPN will assist with clinical procedures, patient education, care coordination, and documentation in compliance with FQHC standards and regulatory requirements. Key Responsibilities Assist providers with examinations and treatment of patients across the lifespan Perform routine clinical procedures including: Vital signs, Phlebotomy, Injections, immunizations and medication administration Conduct patient intake and clinical assessments Provide patient education on medications, chronic disease management, and preventive care Perform point-of-care and basic laboratory testing Ensure compliance with HIPAA, HRSA, and organizational policies Contribute to quality improvement initiatives and patient-centered care efforts Support clinic flow and assist with additional clinical duties as needed to ensure continuity of care Preferred Qualifications Experience in Family Medicine, Internal Medicine, or primary care Proficiency with EHR systems Strong clinical assessment and patient care skills Excellent communication skills and ability to work with diverse patient populations Commitment to serving underserved communities If you are passionate about community health and making a meaningful difference in the lives of patients, we encourage you to apply and become part of our dedicated healthcare team.
