Part-Time /Per Diem Schedule:
Monday-Friday, with every other weekend requiredEstimated Annual Earnings:
$39,000-$55,800+ , depending on patient volume and weekend visits This is an excellent opportunity for an LPN seeking flexible part-time or per-diem work while providing meaningful one-on-one care to patients in the community. Responsibilities Provide skilled nursing care to patients in their homes. Perform nursing assessments and monitor patients for changes in condition. Administer medications and treatments as ordered. Follow individualized plans of care. Educate patients and families regarding medications, treatments, and disease management. Communicate changes in patient condition to the appropriate healthcare professionals. Maintain accurate and timely clinical documentation. Collaborate with RNs, physicians, therapists, and other members of the healthcare team. Provide compassionate and professional patient care. Schedule Requirements Part-time and per-diem opportunities available. Primarily Monday-Friday. Must be available to work every other weekend. Flexible scheduling based on nurse availability and patient needs. Opportunity for additional visits based on patient census. Qualifications Current, unrestricted LPN license. Current CPR/BLS certification. Previous home health or community nursing experience preferred. Reliable transportation. Ability to travel to patient homes.
