Job Description

We're looking for a compassionate licensed practical nurse to join our team! We're a Home Health Agency headquartered in Hyannis, MA, providing nursing and patient care services to critical regions throughout Massachusetts. We take a sensitive approach to healthcare, meeting patients where we can support them most - at home. Great Benefits! This position is compensated using a pay per visit model. On average our clinicians see between 2 and 3 patients an hour earning between $58 - $87 per hour (calculated at $29 per visit).
OVERVIEW
Relief Home Health Services, Inc. consists of a dedicated Psychiatric Home Health team working with patients with multiple co-morbidities including but not limited to: substance abuse, severe psychiatric issues, homelessness, and complex medical diagnoses. You will work with a comprehensive team, including collaborative community resources, whose goal is to reduce hospital utilization and improve patient outcomes.
JOB SUMMARY A
Licensed Practical Nurse provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families. Professional skilled nursing is performed under the supervision of the Clinical Manager and Director of Clinical Services.
DUTIES/RESPONSIBILITIES
Provide professional nursing services within the scope of nursing practice standards Collaborate with other medical professionals to educate patients and their families Prepares & submits timely, clinical and progress notes within 24 hours of visit Oversees coordination of patient services Follows individualized Plan of Care (POC) and recommends necessary revisions based upon patient needs Recommends "other" professional services that may be appropriate to the needs Effectively communicates ongoing with patient and family progression/changes in POC Effectively communicates with Supervisor/Nursing Director/other disciplines in the case Actively communicates in the case conferencing sessions to establish best practices Coordinates Community Services for the patient to assist in safe home care need Ordering wound care supplies, and oxygen related supplies Prepares patient medication planners per orders, & retrieves medications from the pharmacy when appropriate. Case by case basis may be shared with RN Other duties as assigned by RN including, but not limited to, obtaining verbal orders, office visit notes, completing medication reconciliation Maintains weekly visit Quota as outlined in the job acceptance letter
SPECIAL EQUIPMENT TO BE OPERATED
Vehicle to attend community and other meetings Office equipment: computer, copier, scanner, fax, phones Possible transfer/lifting devices, medical supplies
WORK ENVIRONMENT
Indoors, performing patient care assessment/tasks in the patient's residence Travel to patient residences/meetings; will require transportation to get to various work sites Possible exposure to communicable diseases
FUNCTIONAL ABILITIES
Able to hear, speak, understand and communicate effectively in English, or a patient's native language with interpreter services. Must be able to lift and stoop effectively so as to be able to perform the above listed responsibilities. Able to access patient's homes that may not be handicap accessible.
PHYSICAL REQUIREMENTS
Daily:
walking, standing, sitting, reaching, use of telephone, use of computers & other office equipment, transferring 150 lbs or greater, bending, stooping, climbing stairs. When duties/responsibilities change, the job description will be reviewed and subject to changes of business necessity.
QUALIFICATIONS
Graduate of an approved professional nursing program Currently, licensed "in good standing" in MA Minimum of one (1) year of experience as a professional nurse, preferably in-home care Must have a valid driver's license and current automobile insurance Must have a criminal background check Must have current CPR certification Must be able to read, write and communicate in English
