Job Description

At the Center for Human Development (CHD) , Care Finds a Way. We are looking for compassionate Behavioral Health Licensed Practical Nurses (LPNs) to join our team at Heron House, an 18-bed Enhanced Medical Group Living Environment (EMGLE). Enjoy working with a supportive team of LPNs, CNAs, and Direct Care staff with a low LPN-to-client ratio!
Available Opportunities:
We are currently hiring for 24-hour part-time schedules operating on a two-week rotation for the following shifts: 1st
Shift:
7:00 AM - 3:00 PM (24 hours/week) 3rd Shift /
Overnights:
11:00 PM - 7:00 AM (24 hours/week)
Pay Rate:
$38.50 / hour (+$2.00/hr differential for 11pm-7am overnight shifts)
Bonuses & Perks:

Major Holidays Location:
East Longmeadow, MA Key Responsibilities:
Medication & Care:
Administer and manage medications, assist with audits, and provide adjunct care (dressings, ice/heat treatments).
Direct Care & ADLs:
Assist clients with hygiene, showering, travel to the bathroom, serving meals, and ambulation.
Health Monitoring:
Track vital signs, weight, intake/output, and provide clear observations to the Charge Nurse or RN.
Care Coordination:
Attend care plan/resource meetings and participate in a monthly on-call rotation (stipend provided + hourly wage if called in) .
Qualifications:
Licensure:
Active MA LPN License.
Logistics:
Valid driver's license. Ability to meet physical demands (lifting 45+ lbs, extended walking/standing). Experience (Preferred): 1+ year as an LPN; prior experience in Behavioral Health or working with elderly individuals and adults with disabilities/cognitive challenges.
Requirements:
Ability to complete assigned CHD and Department of Mental Health (DMH) trainings.
