We're looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. In this role, you'll provide hands-on nursing care to older adults in a supportive and collaborative environment. You'll work under the direction of the Program Director and Registered Nurse (RN) and help supervise program aides involved in participant care.
Qualifications:
Valid LPN license from the Massachusetts Board of Registration in Nursing. Certified by a Massachusetts-licensed physician confirming good health and freedom from communicable disease (including tuberculosis) prior to employment. Experience working with older adults and individuals with cognitive impairments is a plus. Ability to work flexible or unscheduled hours as needed. Strong teamwork skills and a commitment to quality care.
Responsibilities:
Monitor participants' health and report changes to the Program Director. Record vital signs and weights. Deliver nursing care and treatments as directed by the RN. Assist in documenting care, including monthly nurses' notes and health education (if delegated). Provide skincare and nail care to participants. Follow infection control practices. Prepare diet orders and oversee meal service based on participants' dietary needs. Report concerns with food or fluid intake. Assist with emergency care when needed. Supervise program aides assisting with nursing services.
