License Minimum 3 years of recent LPN/LTC experience Current

BLS/BCLS

certification Charge Nurse experience preferred

Roles & Responsibilities:

Provide direct patient care within LPN scope. Monitor residents, record vital signs, perform assessments. Administer medications, injections, treatments, and wound care. Assist with ADLs (bathing, grooming, feeding, mobility) Coordinate CNA assignments and provide supervision/support to nursing staff Maintain accurate EMR/EHR documentation.
