Job Description

Hebrew SeniorLife, a Boston-based nonprofit senior care organization affiliated with Harvard Medical School, is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care for older adults. In this role, you will deliver hands-on nursing care, administer medications, monitor resident conditions, and document in EMR systems. You'll collaborate with an interdisciplinary team to support care planning, rehabilitation, memory care, and hospice services. Responsibilities Provide direct nursing care to senior residents, including medication administration and treatments per provider orders Monitor, document, and report changes in residents' conditions using EMR systems Support RNs and providers in care planning, assessments, and interdisciplinary care conferences Educate residents and families on care plans, medications, and health maintenance Promote safety, dignity, and independence for residents in alignment with organizational values Collaborate with an interdisciplinary team to deliver coordinated, person-centered care Follow infection control, regulatory, and quality standards for skilled nursing and senior living Assist with admissions, discharges, and transitions of care for residents Required Skills Medication administration Wound care Vital signs monitoring Geriatric nursing Electronic medical records (EMR) Infection control Patient and family education Care planning support Documentation and reporting Basic life support (BLS)
