Responsibilities:
Provide direct patient care in a variety of settings, including skilled nursing facilities, home health care, and assisted living communities Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams Conduct initial and ongoing assessments to identify patient needs and develop effective care strategies Administer medications, treatments, and therapies as ordered by physicians Collaborate with healthcare teams to coordinate patient care and services Maintain accurate and detailed records of patient care and treatment Participate in ongoing professional development and continuing education to stay current with best practices and industry advancements
Requirements:
