We are seeking a compassionate and experienced Telephone Triage LPN to join our team! In this role, you will provide remote nursing assessment and support by phone, helping patients determine the appropriate level of care, offering health advice, and coordinating next steps in their care journey. You will play a key role in ensuring patient safety, efficient care delivery, and patient satisfaction.

Key Responsibilities:

Conduct thorough nursing assessments over the phone using clinical protocols and critical thinking skills. Triage patient symptoms and determine urgency based on established guidelines and patient history. Provide health education, advice, and home care instructions within LPN scope of practice. Document all patient interactions clearly and accurately in the electronic health record (EHR). Collaborate with RNs, physicians, and other healthcare providers as needed for high-acuity or complex calls. Schedule appointments, direct patients to appropriate care settings, or initiate emergency services if necessary. Follow up with patients as required to ensure resolution of concerns or symptoms. Maintain current knowledge of clinical protocols, procedures, and best practices in telephonic nursing care.

Qualifications:

Active LPN license in Massachusetts Minimum 2 years of clinical nursing experience Excellent communication and listening skills Strong clinical judgment and ability to work independently Proficiency with EHR systems and telephone systems Comfortable using clinical decision support tools or triage software

From $35.00 per hour
