Position Summary The Licensed Practical Nurse (LPN) provides direct resident care and supervises Certified Nursing Assistants in accordance with physician orders and individualized care plans. Working under the direction of the RN Shift Supervisor, the LPN helps ensure residents receive safe, compassionate, and high-quality nursing care. What You'll Do Provide direct nursing care and treatments as ordered by physicians. Administer medications accurately and safely while maintaining proper documentation. Assist with developing, implementing, and updating individualized care plans. Monitor resident conditions and promptly report changes to the RN Supervisor and physician. Supervise, guide, and support nursing assistants in providing quality resident care. Maintain accurate, timely, and compliant nursing documentation. Build positive relationships with residents and their families through compassionate communication. Follow HIPAA regulations, infection prevention protocols, and facility safety policies. Participate in interdisciplinary rounds and respond appropriately to resident emergencies. What We're Looking For Current, active Connecticut Licensed Practical Nurse (LPN) license in good standing. Knowledge of nursing practices and medical procedures in a long-term care setting. Strong assessment, observation, and documentation skills. Excellent communication and interpersonal abilities. Compassionate, dependable, and team-oriented. Long-term care or hospital experience is preferred but not required. Working Environment This position requires frequent standing, walking, bending, lifting, pushing, pulling, and assisting residents throughout the shift. Team members may be exposed to infectious materials, cleaning chemicals, and other healthcare-related environments while following all safety and infection prevention guidelines. Flexibility to work weekends, holidays, and additional hours as needed is required.
