Cape Heritage Rehabilitation & Health Care Center in Sandwich Up to a $5,000 sign-on bonus available! Hourly rate ranges between $40 and $46, depending on the shift. Evenings, overnights, and weekends have shift differentials. 40 hours available on 11p-7a, includes an every other weekend rotation. Per diem available on all shifts.Job Summary:
As an LPN, you will provide direct nursing care under the supervision of the RN to residents and supervise the day-to-day nursing activities performed by the nursing assistant to ensure that the highest degree of quality care is maintained at all times.Responsibilities:
Meet the nursing and rehabilitation needs of each resident Maintain current resident care plans on all residents under your care Follow physician's orders and render/assist with complete nursing care, treatments, dressings, and other physician orders Transcribe physician's orders promptly and accurately Review patient records daily to assure accuracy and completeness Document comprehensive and complete nursing notes Visit each resident daily to evaluate resident needs Maintain accurate awareness of residents on alert list Document and report any unusual or significant findings; contact the physician when necessary Administer medication according to procedure Initiate any and all emergency procedures as necessary Demonstrate correct safety techniques Respect and take care of facility equipment and not wasteful of supplies Contribute to effective communication and pleasant working conditions Contribute to and promote positive resident and family relations Comply with facility Privacy Practices Policies & Procedures, protecting residents' individual health informationRequirements:
Graduate of an approved LPN/LVN program CPR certified Active and unencumbered LPN/LVN license in Massachusetts, or be in the process of obtaining one New graduates welcome!Additional Requirements:
