Our Hospice Residence is located in Wayland and was reopened in the Fall of 2022. This is an exciting opportunity for LPNs interested in providing end-of-life care at a new 10-bed hospice facility. This position is eligible for a $5000 sign-on Bonus.
Job Responsibilities:
This position provides direct patient care, including ongoing assessment, pain management. and symptom control in line with the plan of care established for the patient. The Licensed Practical Nurse notifies the Nursing Care Coordinator of any significant changes in the patient's condition or any changes that necessitate physician notification and documents all care provided, including clinical observations and interventions.
Schedule and Weekly Hours:
12-hour shifts, days and nights available. Weekends and/or weekdays.
Education, Skills, and Experience:
Licensed Practical Nurse (MA license) A minimum of two years prior experience in community health, long term care facility, oncology, or a related field preferred. Ability to be a team player and work harmoniously with patients, families, co-workers, and management. Providing care consistent with written instructions. Providing assessments of the impact of the terminal diagnosis on the patient's physical, functional and environmental needs and activities of daily living. Providing on-going evaluation of patient/family response to care and informing the Nursing Care Coordinator and other personnel of changes in the patient's needs and outcomes of interventions. Preparing documentation that demonstrates adherence to the patient's individualized plan of care and progress towards established goals. Assessing for patients' safety. Providing personal care including assistance with daily hygiene, ambulation, exercise, toileting, nutritional support, dressing changes and other care as designated by the Nursing Care Coordinator. Assisting with medications that are ordinarily self-administered. Assisting patients with household activities. Pay Range:
$34-41/hour with a differential of $2 per hour for evening shifts and $3 per hour for weekend shifts: This number reflects an average range for this position. Individual pay is determined through a variety of factors, including experience, education, certifications, and skills, and can exceed the range.
