Join our team at South Mountain Rehab Center, a warm and home-like environment where personalized care is our top priority. As a Licensed Practical Nurse (LPN), you will provide supervision of staff and safeguard the health, safety, and welfare of all residents under your care. We're looking for a dedicated individual with a passion for delivering high-quality care to join our Skilled Nursing team.Responsibilities:
Deliver safe, timely, and effective nursing care to residents under the direction of an RN or physician Monitor patients' health, administer medications, and provide wound care and other treatments Collaborate with interdisciplinary teams to develop and update individualized care plans Support residents and their families with education, comfort, and reassurance Maintain accurate documentation and comply with regulatory standards Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team Observes safety policies and reports any safety concerns Demonstrates respect of resident rights and dignity Fulfills the role of charge nurse when assigned Supervise and support CNAs in the delivery of care Maintain a clean, safe, and therapeutic environment for residents Ensure compliance with infection control, safety, and HIPAA policies Participate in quality assurance activities and staff meetingsRequirements:
Active LPN license in Maryland or Multistate Experience in long-term care or skilled nursing preferred- new grads welcome!
