On-site, residential facility Position Summary The Licensed Practical Nurse (LPN) provides direct nursing care and medication management for adults in a residential dual-diagnosis treatment program. Working under the direction of an RN and in coordination with the clinical and medical team, the LPN supports residents' physical and behavioral health, oversees safe medication practices, and helps maintain a safe, therapeutic environment — in accordance with the Maryland Board of Nursing scope of practice, CARF standards, and facility policy. Key ResponsibilitiesNursing & Medication Management Administer medications and treatments accurately and on schedule, and document per policy. Monitor residents for therapeutic effects, side effects, and changes in condition; report concerns to the RN and prescriber. Provide oversight and delegation support for Certified Medication Technicians (CMTs) as permitted by the Maryland Board of Nursing. Maintain accurate medication records, counts, and secure storage in compliance with regulations. Assist with medication reconciliation at admission, transfer, and discharge. Resident Health & Monitoring Obtain and record vital signs, and conduct basic health screenings and observations. Support management of withdrawal symptoms and chronic conditions in coordination with the RN, prescriber, and clinical team. Recognize and respond to medical or behavioral emergencies, initiating appropriate protocols. Support intake health screening and ongoing wellness checks. Documentation & Compliance Maintain accurate, timely, and complete clinical documentation in accordance with CARF standards, state regulations, HIPAA, and 42 CFR Part 2. Practice within the LPN scope of practice and under required RN direction. Support accreditation, audits, and continuous quality improvement (CQI) activities. Team Coordination & Resident Support Communicate resident status and changes to the nursing, clinical, and residential teams. Participate in shift handoffs, care coordination, and interdisciplinary communication. Provide compassionate, trauma-informed, and dignity-first care and health education to residents. QualificationsRequired Active, unrestricted Maryland Licensed Practical Nurse (LPN) license in good standing (Maryland Board of Nursing). Current CPR/BLS certification. Ability to administer medications and perform nursing tasks within the LPN scope of practice. Strong observation, documentation, and communication skills. Preferred Experience in behavioral health, substance use, residential, or long-term care settings. Experience overseeing or delegating to medication technicians (CMTs). Familiarity with medication-assisted treatment (MAT) and psychiatric medications. Understanding of CARF (or comparable accreditation) standards.
