Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our evening shift team. As a LPN at Autumn Lake Healthcare at Long , you will have the opportunity to make a meaningful difference in the lives of our residents. Job Responsibilities:
- Provide direct patient care to our residents, including medication administration, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with our interdisciplinary team
- Maintain accurate and up-to-date records of patient care and treatment
- Report any changes or concerns to our management team
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Qualifications & Experience Requirements:
- Previous experience as a Licensed Practical Nurse (LPN) is preferred
- Must hold a current Licensed Practical Nurse (LPN)
License Schedule:
