Immediate / As soon as credentialed Job Responsibilities As an LPN, you will provide safe, compassionate, and high-quality nursing care in accordance with accepted nursing standards.

Responsibilities include:

Provide patient/resident care in accordance with accepted standards of nursing practice, the written plan of care, and the physician's plan of treatment. Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team. Administer medications and prescribed treatments in accordance with the plan of care and facility policies. Monitor residents' conditions and promptly report changes to the supervising nurse or physician. Respond appropriately to residents' clinical and healthcare needs. Collaborate with DSPs, nurses, program staff, families/guardians, physicians, and other members of the interdisciplinary team. Follow all applicable DDA, healthcare, medication-management, safety, and agency policies and procedures. Maintain accurate and timely nursing documentation in accordance with physician orders, care plans, and regulatory standards. Promote a safe, respectful, and therapeutic environment for residents and their families. Work collaboratively with the healthcare and interdisciplinary team to support quality resident care. Ethical and Professional. Ensures Patient Safety and Quality Care, Builds Patient Trust, Fosters Effective Collaboration with the Team, Upholds Ethical Standards, and Drives Patient Advocacy. Pay close attention to detail, follow evidence-based standards, and remain deeply accountable for their actions to prevent errors. Qualifications Active compact LPN license. Valid BLS/CPR certification required. Ability to provide care for and manage multiple patients/residents in a fast-paced healthcare environment. Strong clinical judgment and decision-making skills.
