The licensed practical nurse performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a licensed health care provider.RESPONSIBILITIES
Collects patient data related to patient and family needs Performs tasks related to the comprehensive plan of care involving the patient and the family. Performs and documents direct patient and family centered care according to nursing standards of practice. Responds to patient conditions to achieve positive patient outcomes. Performs tasks related to discharge planning to ensure safe and appropriate transition of care. Supervise unlicensed team members in the delivery of care. Provides world class customer service to patients and families.REQUIRED QUALIFICATIONS
Graduate of an accredited practical nursing program Current, unrestricted New York State licensed practical nurse license.PREFERRED QUALIFICATIONS
One year of post-licensure practice as a Licensed Practical Nurse preferred.UNION:

PHYSICAL REQUIREMENTS
Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20-50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Any physical requirements reported by a prospective employee and/or employee's physician or delegate will be considered for accommodations.PAY RANGE
$30.16 - $42.30 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
