Job Description

Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our clinic team. The LPN will provide high-quality patient care, assist with clinical procedures, and support the overall functioning of the healthcare facility. This role offers an opportunity to work in a dynamic environment where patient safety, confidentiality, and well-being are prioritized. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional care across diverse patient populations. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications and injections accurately following established protocols, including IV infusions when applicable. Assist physicians and other healthcare providers during examinations, procedures, and treatments such as catheterization and suturing. Manage patient intake and discharge processes while maintaining detailed medical documentation in electronic health record (EHR) systems like Epic or Cerner. Provide wound care, including dressing changes and sterile processing procedures. Support patients with chronic conditions such as dementia, Alzheimer's disease, or developmental disabilities through tailored care plans. Conduct specimen collection and processing for laboratory testing while adhering to infection control standards. Educate patients and caregivers on health management topics including medication administration, nutrition, pain management, and post-discharge care. Ensure compliance with HIPAA regulations and clinical confidentiality policies at all times. Collaborate with interdisciplinary teams to coordinate case management, discharge planning, and long-term care strategies.
