One-on-one patient care in the home setting Supportive and collaborative clinical team Opportunity to build meaningful patient relationships Competitive compensation package Free continuing education and professional development opportunities Career advancement opportunities Medical, dental, and vision insurance 401(k) with company match Family and pet bereavement benefits A mission-driven culture focused on compassion and excellence What You'll Do Deliver skilled nursing care under the direction of a Registered Nurse and physician Follow and implement individualized Plans of Care Monitor patient conditions and communicate changes to the RN Case Manager and care team Educate patients and caregivers on disease management, medications, and self-care techniques Document assessments, interventions, and patient progress accurately and timely Promote patient safety and implement strategies to reduce avoidable hospitalizations and emergency room visits Collaborate with physicians, nurses, and interdisciplinary team members to ensure quality patient outcomes Support patients and families with compassionate, respectful, and patient-centered care Identify opportunities for improvement and communicate recommendations to the RN Case Manager What You'll Bring Graduate of an accredited Associate Degree Nursing program Current state LVN/LPN license Minimum of one (1) year of clinical nursing experience required Home Health or Hospice experience preferred not required Strong communication, critical thinking, and patient care skills Ability to sit, stand, bend, lift, and move throughout the workday, including lifting up to 50-100 lbs. when necessary Valid driver's license, reliable transportation, and current auto insurance Willingness to travel within the assigned service area Why Nurses Love Home Health Home health offers the opportunity to slow down, focus on quality patient care, and build lasting relationships with those you serve.
