Responsibilities Conduct thorough patient assessments including vital signs, physical examinations, and review of medical histories using Electronic Health Record (EHR) systems. Administer medications and treatments accurately, including IV infusions, injections, catheter care and tube feeding while adhering to HIPAA and infection control policies. Monitor patients' conditions closely, managing acute pain and respiratory needs with airway management techniques or ventilator support when necessary. Document all patient interactions meticulously in medical records, ensuring compliance with long-term care regulations and ICD coding standards. Collaborate with physicians and other healthcare professionals in triage situations, emergency responses, or when coordinating complex case management plans for patients with dementia or behavioral health needs. Provide compassionate patient care tailored to geriatric populations or individuals with developmental disabilities; support memory care initiatives including Alzheimer's disease management. Assist with diagnostic evaluations such as specimen collection/processing or medical imaging procedures when required. Experience Proven nursing experience in acute care settings such as ICU, emergency room, Level I or Level II trauma centers, or hospital medicine units. Demonstrated expertise working with diverse patient groups including geriatrics, pediatrics, neonatal ventilator management, or individuals with disabilities. Hands-on experience in long-term care facilities like skilled nursing facilities or assisted living communities is highly valued. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation and record management. Strong knowledge of physiology, anatomy, pharmacology related to pain management and infection control practices. Experience working within hospice care or post-acute settings is a plus; certification in Basic Life Support (BLS), CPR, and first aid is required.
